We as adults can have a variety of schemas; as an example we can have the schema of winning, that is accomplished through out past experiences as we try hard and win. Imagine this person had always had the schema and has tried hard and won the situation, but this time he fails in a match as a result of complicated sitution and lots of other variables that can be effective. So his winning schema does not work as his previous experiences. This is a failure of assimilation, now if he wins the match this time this can be an approval to his previous schema and the assimilation approves and happens. Now in turms of failure, the person needs to change his previous schema and adapt it to the new situation. Through this, new ideas come up but there is a change in his last winning schema.
