If an alcoholic is given a drink alongside a sniff of some foul smelling substance, for example, eventually he/she will associate alcohol with the foul smell, and be deterred from drinking.
Regarding phobias, exposure therapy works through slowly demonstrating to the therapy recipient that their phobia is irrational by incrementally putting them in situations involving the thing of which they are scared. After consistently being shown that they are not harmed though exposure to their fears, they eventually begin to believe it not to be harmful.
